DSRB Templates for Study Documentation

The following templates are provided as a guide for Principal Investigators to create their own study documents. These templates contain the critical elements which are required by the DSRB. While it is not compulsory to use these templates, Principal Investigators are encouraged to do so.

The following softcopy DSRB Forms are provided for Principal Investigators to use and submit to the DSRB in the event that ECOS is not available (e.g. during system downtime). The DSRB will not accept the softcopy forms for review in any other circumstances unless otherwise stated.
